You will be focused on delivering comprehensive, patient-centered care across the health care continuum, as a Licensed Clinical Social Worker you will work closely with DCMG primary care physicians to deliver integrated member care, clinical coordination, coaching, consultation, and intervention with a particular focus on patients with a need for placement and care transitions and patients with behavioral health needs which require referrals and follow up. You will leverage technology and work closely as part of a physician-led, interdisciplinary care team, as the Licensed Clinical Social Worker you will lead the development, implementation and ongoing monitoring of program and quality initiatives to address the behavioral health needs of patients.
Your responsibilities consist of:
• You will apply driven methods for identifying, screening, and assessing patient's needs, including the identification and risk stratification of complex, co-morbid patients, with the objective of focusing care management efforts on the segment of the population requiring the highest degree of support with a spotlight on care transitions and behavioral health needs.
• Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures needed. Recommends community resources to meet patient and family needs.
• Triaging patient telephone calls and provides consultation.
• Identifying symptoms and signs of psychiatric disorders; comprehensively assess, diagnose, and treat symptoms.
• Preparing clinical reports in electronic health record to include diagnoses, prognoses, and treatment plans.
• Implementing and providing new treatment, training, and rehabilitation programs.
